Runbook v3: debugger-conflict fix, verified attach dance, version-gated server

Verified live in Flow: + button is an unlabeled add_2 icon button; picker is a
role=dialog whose character list is PORTALED outside it; Add to Prompt yields
img[alt='Character reference image'] chips. flowext updated accordingly (chip-
verified attach, ext=3 version gate so a stale content script gets 204s, requeue
notes logged). MeshGod uvicorn squatting 8017 evicted (its real port is 8010).
3 damaged tasks re-queued under fresh ids.
